Write up manual testing procedure for BF-CPP and BF-ITK

Description
The BF-CPP and BF-ITK projects need to be tested on major OSes prior to release (see ticket #8558). However, before we can do that, we need a document that spells out the exact steps to perform.

We reviewed the web sites (BF-CPP and BF-ITK) as well as the FARSIGHT Bio-Formats pages (1, 2), and all looks good. We just need to update the BF-CPP readme files (readme.txt, readme-windows.txt, readme-macosx.txt, readme-linux.txt).

Updated the BF-CPP instructions in each platform's readme.txt. Opened https://github.com/openmicroscopy/bioformats/pull/76 to incorporate these changes.
BF-ITK instructions were updated with additional information differentiating Windows and OSX/Linux builds. These instructions can be found at: http://loci.wisc.edu/bio-formats/itk
