Implement reference counting on glacier sessions

Description

Currently, connecting to a running session is dangerous, in that another session could timeout if the client has died.

r2592 to r2598 close this issue (even if they are listed as #1018). Carlos and Ola, this should mean that it is now possible to attach from one session and not worry about another session killing you, since the references are counted. Just how to do this will be updated on OmeroSessions.
