Bug: Hamamatsu VMS files have incorrect dimensions

While all of the files are picked up for the CMU-{1, 2, 3} datasets, the width and height are only set to match the first tile. This should be changed to match the width/height of the tiles after they have been stitched together.

This is going to require a near rewrite of VMS support; scheduling for 5.2.0 given the breaking nature.

Further investigation needed to know just how bad a reworking of VMS support will be (we have a lot more pathology format experience than we did a year ago). I'll either fix or have more detailed comments for what needs to happen before end of March.
