Comment: these stacks are from a 3 channel z-series exported from Slidebook 6 as 16-bit, without the OME-XML option selected. Bio-formats 5.0.5-DEV rev f660e3c imports a merged stack with 2 timepoints and duplicating the slices.
this does not happen when the user exports from Slidebook with the OME-XML option checked.

I haven't been able to reproduce this at all with f660e3c or current develop - choosing any one of the files results in a 3-channel stack with no duplicate images. SizeT is set to 32 and SizeZ is set to 1; there isn't much we can do about that though, as there is no metadata in the files to distinguish Z from T.
The only two things I can think of are that 1) a second older version of Bio-Formats is installed, or 2) there were additional files in the folder that were not uploaded to QA and which interfere with the dimension detection.
Moving to 5.1 for initial investigation, feel free to move.
