Bug: Insight create user UI requires a group allocated, but UI doesn't show it as required

Description

When using the (5.1.4) Insight Create User dialogue, there are a number of required fields. With each of the required fields filled out, the user still can't press 'create'.

There is no UI element showing that a group allocation is required, 'create' is just greyed out. This doesn't feel correct, since the user has completed all apparently required fields.
