Make OriginalFile path+name be UNIQUE in repositories

  • Definition of path is dirname filepath
  • Definition of name is basename filepath
  • Where possible (e.g. original_metadata.txt) we will correct path/name usage
  • A unique, partial index will be added on (repo, path, name) where repo is not null
