Limitation: Attachments shown as "none" even when there is one

Description
I selected two images.
Attachment -> upload local document. Choose file, and "Attach"
It looks like the step has just failed as the side bar refreshed itself and says attachments "none"
The files have been attached ok and can be seen by selecting the individual images.
