Remove reliance on single rendering definition

Description
The reliance on single rendering definitions should be removed as should the references to "IMG_RDEFNS" and annotation loading during thumbnail retrievals.

New/changed methods in the gateway:
- _getRDef() -- Now provides "custom" logic prior to initialization of a thumbnail store or a rendering engine.
- _onResetDefaults() -- "Callback" that is called after a successful execution of resetDefaults() during the initialization of a thumbnail store or a rendering engine.
Bug fixes:
- Fixed issue where thumbnail store was not resetting the missing pyramid state.
Every attempt was made to preserve the existing custom logic that is in
place while utilizing the new getRenderingDefId() methods on the
thumbnail store and rendering engine, keeping in line with the
permissions changes introduced in Beta 4.2.x and missing pyramid
handling introduced in Beta 4.3.0.
Chris, any estimate on time for this?
