Tested this extensively with a variety of big image sizes (up to 20k by 15k) and cannot reproduce either using the Windows server or a local server running on Mac OS X. I anticipate this issue was really down to all the synchronization and threading issues we saw on Windows prior to Carlos' resolution of multi-threading problems on Windows under IIS.
