Bug: Insight hangs when double clicking on invalid image thumbnail

Description
Import a big image (8kx8k.jpg) from the command line. As soon as it is imported refresh the dataset and double-click on the temporary thumbnail. The "image not ready" dialog is displayed but closing it leaves the "Opening Image Viewer..." dialog. If this is cancelled it reappears straight away (repeatedly) and the only way to resolve the problem if to Force Quit Insight.
This also happens with the black thumbnail of an invalid image (screenshot for this attached).
I have also reproduced this on the fslite-stage2 branch where all images go through the PixelDataHandler?.

Review the Cancel option
Close the loading dialog when the re cannot be loaded.
