Doc: fix developers TOC hierarchy

Currently the main index TOC links to the first page in each Chapter of the developer docs.
However, these Chapter landing pages do not contain links to other pages in the chapter. This means that you can't navigate to them and they don't even show up in the pdf export!
The solution is to only have a single developers link on the main index page - this takes you to the developers TOC with every page listed.
Also need to update the title of the developers TOC page to remove 'OMERO', since you then have a breadcrumb of
OMERO UNKNOWN documentation » OMERO Developer Documentation
