Bug: Metamorph reader does not cope with multiple channels having the same name

Description
See dataset in metamorph/ben/Duplicate Channel Names/.
The file for the third channel isn't even picked up by the used file list, which suggests a problem in the file grouping and not an indexing problem.

Fixed with: https://github.com/melissalinkert/bioformats/commit/5d495828bd1b49db6d4fd7a3691df9b9de3dd3fb
This actually had nothing to do with the two channels having the same name. The problem was that the channels alternated between having and not having Z sections.
